Payment Options – PPO Insurance

You can use your PPO insurance to pay for surgery with Dr. Oliak if your policy covers weight loss surgery and if you meet your policy’s criteria for surgery.

Will My PPO Cover Weight Loss Surgery?

Weight loss surgery is often a covered benefit, but not always. We will do a free insurance verification to check your insurance benefits before your consultation.

Are There Out-of-Pocket Costs?

Yes, always. There are co-pays, co-insurance, and deductibles that must be paid out-of-pocket. Out-of-pocket costs vary over a wide range (average 3k-10k) depending on insurance coverage. We will provide you with an estimate of all out-of-pocket costs on the day of your consultation.

How Do I Get an Insurance Authorization for Surgery?

If you meet your insurance policy’s criteria for surgery, we will schedule you for the required tests/evaluations and obtain an authorization for weight loss surgery.

Payment Options – Private Pay

For patients not using health insurance to pay for weight loss surgery, we offer competitive weight loss surgery self-pay prices, and attractive financing options through CareCredit.

How Much Does Weight Loss Surgery Cost?

Weight loss surgery prices vary depending on a number of factors, but a rough range is approximately $15-20k all-inclusive (surgeon, assistant, anesthesiologist, hospital, follow-up, etc). We will give you a more exact quote on your consult day after you meet with Dr. Oliak.

Is Private Pay a Common Payment Method?

Yes. Common reasons for patients to choose private pay include lack of insurance coverage, onerous preauthorization requirements (e.g. 6 month medically supervised diet), and undesirable insurance requirements for who the surgeon must be (e.g. HMO’s). Approximately 30% of our patients have been private pay every year since 2007. This percentage has increased to around 50% over the past several years as insurance coverage for weight loss surgery has unfortunately become more restrictive.
